29 CFR 1904.29(b)(5): The Log of all Work-Related Injures and Illnesses (OSHA Form 300) forms were maintained on a computer that could not produce equivalent forms when they were needed, as described under 1904.35 and 1904.40: a) On or about 9/7/17, VA Medical Center, Birmingham, AL, the OSHA 300 logs for calendar years 2017 and 2016 were provided with all injured employees' names removed from the log. The standard allows only specific injuries listed in 1910.29(b)(7) to be removed and identified as privacy cases.
