Chambers Home Improvement LLC on or about September 7, 2017 at the address of 232 Elm Street, Findlay, Ohio, 45839: The employer failed to ensure an accident prevention program was in place to aid the employer and his employees in the recognition and avoidance of hazards associated with residential roofing operations (i.e.; fall protection, and use of personal protective equipment).

One employee was exposed to falls while using the ladder to access the roof.
