Chambers Home Improvement LLC on or about September 7, 2017 at the address of 232 Elm Street, Findlay, Ohio, 45839: The employer failed to ensure an accident prevention program was in place to aid the employer and his employees in the recognition and avoidance of hazards associated with residential roofing operations (i.e.; fall protection, and use of personal protective equipment).

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  a) D.L. Chambers Home Improvement LLC on or about September 7, 2017 at the address of 232 Elm Street, Findlay, Ohio, 45839: The employer did not ensure employees were protected from falls while installing floor joist to the second level deck of a residential type structure.  The employees were exposed to a potential 11 foot fall to the level below.

Chambers Home Improvement LLC on September 7, 2017 at the address of 232 Elm Street, Findlay, Ohio, 45839: The employer did not ensure the side rails of the 24 feet fiberglass extension ladder was extended at least three feet above the landing surface. One employee was exposed to falls while using the ladder to access the roof.
