This was most recently documented at 940 Calvert Street, Lincoln, NE 68502 on or about September 6, 2017 when employees were observed operating the Sky Trak 80421 telehandler forklift without successfully completing formal instruction on the safe operation of powered industrial trucks.

This was most recently documented at 940 Calvert Street, Lincoln, NE 68502 on or about September 6, 2017 when employees were observed working in the grain elevator where more than 1/8 inch of grain dust had accumulated on floors and horizontal surfaces in basement level located within 35 feet of the bucket elevator leg.
