There were two lathes (Harrison and Tarnow) that were not guarded at the rotating chucks.     c. There was a Dake horizontal bandsaw that had 13 inches of the unused portion of the blade that was not guarded.    d. There was a DoAll vertical bandsaw that had 3 inches of the unused portion of the saw blade that was not  guarded above and below the work table.    e. There were eight Bridgeport milling machines that were not guarded at the rotating chucks.    f. There was an Accugrid surface grinder with the guard displaced.    g. There was an Allsteel press brake that was not guarded at the point of operation of the dies.

There was a commercial grade multi-tap strip attached to an industrial fan at the paint booth.    b. There were flexible cords attached to two separate electrical junction knockout boxes used for receptacles that were not secured.     NOTE: Knockout boxes were designed to be secured to a fixed/permanent structure. When knockout boxes are used on the ends of flexible cords, employees gripping the box to attach or disconnect a plug may dislodge the circular scored metal portion in the box wall causing it to come in contact with the hot terminal. The result may lead to an electric shock.
