On or about September 12, 2017 the employer had set-up operators that did not control hazardous energies during die changing activities on the mechanical power presses.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about September 12, 2017 the employer had an Insert Molding Machine that had the safety interlocks defeated which exposed the operator to struck-by hazards by the mechanical arm when they went past the barrier guarding. b. On or about September 12, 2017 the employer had a 60 Ton Brudder press that did not have adequate guarding on the sides of the press. The sides of the press had openings for the feeding and discharge of roll stock that were large enough that an employee could reach the pinch point of the dies. c. On or about September 12, 2017 the employer had a Dobby NXT-40 Mechanical Power Press (#994) that did not have adequate guarding on the discharge side of the press. The discharge side of the press had an opening large enough that an employee could reach the pinch point of the dies. d. On or about September 12, 2017 the employer had a Dobby NXT -40 Mechanical Power Press (#995) that did not have adequate guarding on the discharge side of the press. The discharge side of the press had an opening large enough that an employee could reach the pinch point of the dies. e. On or about September 12, 2017 the employer had a Mechanical Power Press (#961) that did not have adequate guarding. The barrier guarding over the point of operation had an approximately 10 inch opening that allowed access to the pinch point of the dies.

29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a. The employer had two forklift boom attachments that were used to hoist and load dies into their machinery without approval from the forklift manufacturer that it was safe to do so.
