29 CFR 1910.28(b)(3)(i)(A): The employer did not ensure that each employee on a walking-working surface was protected from falling into any hole or pit that was 4 feet (1.2 m) or more above a lower level by a cover or covers:     The employer is failing to protect employees from tripping and fall hazards associated with grain elevator housekeeping activities. This was most recently documented, on 09/19/17, at the establishment located at 1892 US Hwy 83, Sublette, Kansas. The employer did not assure covers were readily available for all floor holes on the distribution level of the grain elevator.  Employees engaged in housekeeping activities were exposed to trip and fall hazards into open distribution level floor holes.

29 CFR 1910.272(g)(1)(iv): The employer did not prohibit "walking down grain" and similar practices where the employee walks on grain to make it flow within or out from a grain storage structure, or where an employee is on moving grain:    The employer is failing to protect employees from entrapment and engulfment hazards associated with the cleaning of grain elevator grain bins. This was most recently documented, on 09/14/17, at the establishment located at 1892 US Hwy 83, Sublette, Kansas. The employer did not ensure employees from walking on moving grain while vacuuming grain from the bottom of a grain bin resulting in an employee being engulfed in grain.

This was most recently documented, on 09/14/17, at the establishment located at 1892 US Hwy 83, Sublette, Kansas. The employer did not ensure employees entering a grain bin to vacuum out grain in the bottom of a grain bin utilized a lifeline when walking or standing on grain.

This was most recently documented, on 09/14/17, at the establishment located at 1892 US Hwy 83, Sublette, Kansas. The employer did not provide and ensure rescue equipment such as but not limited to hoisting and winching equipment was readily available prior to employees being permitted to enter into a grain to remove grain from the bottom of the bin.

The employer did not ensure electrical disconnects were properly labeled to indicate their intended use for the equipment listed below.    a) Exterior disconnect for aeration motor for bin #26.    b) Exterior disconnect for aeration motor fan on bin #10.    c) Disconnect for man lift in the MCC room.

In the main work room the employer did not provide a cover for the railcar counter exposing employees to energized electrical wires.
