29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a)  Roof - Employees conducting roofing operations on a steep residential roof donned a body harness as part of a personal fall arrest system but did not attach to an anchor point and were exposed to an approximate 31 foot fall hazard to the ground below, on or about September 18, 2017.  Abatement certification and documentation required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  Job Site - Employees used a 40 foot portable Werner extension ladder to access/egress the roof and the ladder side rails extended approximately 12 inches above the eave, exposing employees to an approximate 31 foot fall hazard, on or about September 18, 2017.  No abatement certification or documentation required.

The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
