29 CFR 1926.95(a): 29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided and not used wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:  On or about 9/19/19, at the overhead power lines across the street from 3279 Elliswood Street, Sulphur, LA; the employer allowed an employee to connect the field phase wire near an energized jumper line, and the employee was not wearing electrical protective sleeves to guard against bodily contact with the energized line.

29 CFR 1926.960(c)(1)(iii)(A): The employer did not ensure that when employees approach exposed energized parts within the employer's established minimum approach distance, the employee was insulated from the energized part by wearing rubber insulating gloves or rubber insulating gloves and sleeves.  On or about 9/19/17, at the overhead power lines across the street from 3279 Elliswood Street, Sulphur, LA; the employer allowed an employee to connect the field phase wire near an energized jumper line and the employee was not wearing rubber-insulating sleeves to prevent contact with the energized line.

On or about 9/19/17, at the overhead power lines across the street from 3279 Elliswood Street, Sulphur, LA; the employer allowed an employee to connect the field phase wire near an energized jumper line without ensuring that the employee also continued to wear rubber-insulating sleeves.

On or about 9/19/17, at the overhead power lines across the street from 3279 Elliswood Street, Sulphur, LA; the employer allowed an employee to remove his rubber-insulating sleeves within the minimum approach distance, while connecting a field phase wire near an energized jumper line and made contact with the energized line.
