29 CFR 1910.269(g)(2)(iv)(B):The employer did not ensure that personal fall arrest systems were used in accordance with subpart I of this part. On or about September 29, 2021, at an overhead power line replacement site located on a rural road in Locust Grove, OK, an employee was allowed to work in the basket of a bucket truck without attaching his lanyard to the bucket anchor, exposing the employee to the hazard of falls.
