29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) Employees who perform servicing, maintenance on or around the Robopac Plastic Wrapping machines 620 and 620-100 were not required to: 1) Physically locate the electrical disconnect in such a manner as to isolate the machine or equipment from the energy source. 2) Affix a lockout or tagout devices to the electrical disconnect in that on 9/13/17 Robopac machine 620 was not locked out and/or unplugged to prevent employees exposure to start up and/or struck by hazards .
