When normally enclosed live parts are exposed for inspection or servicing, the working space, if in a passageway or general open space, shall be suitably guarded. a. On or about 09/19/2017, in the Boiler #2 area, the employer did not ensure sufficient clearance was maintained around the CB-NATCOM Ignition Transformer. A WERNER 20' Extension Ladder was being stored in front of and against the CB-NATCOM Ignition Transformer.
