29 CFR 1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls were not utilized: a) On or about October 19, 2017, an employee performing arc welding was exposed to a noise level of 105 dBA for an eight-hour time weighted average. No engineering controls for noise had been implemented. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i., and then only used with effective chip guarding and personal protective equipment: a) Finishing Cut Off - On or about October 19, 2017, employees, separating stainless steel products, were using shop air, with a nozzle pressure of 55 p.s.i., to clean the dust off of their work clothes and out of their respirator HEPA filters during breaks. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
