29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: On or about 9/20/2017, during commercial construction at 38908 Medina Rd, Fairlawn Ohio, the employer failed to ensure that the Wayco scaffolding was inspected for defects. The employees were working at a height of about 16 feet, and there were no mid rails in place.
