29 CFR 1910.138(a): General requirements. Employers shall select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes. The employer did not require the use of hand protection to protect from pinch point hazards when working on a Kenco KL900 Barrier Lift in that on August 30, 2017 an employee received an amputation to the tip of the left hand middle finger.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. The employer failed to report an amputation which occurred on August 30, 2017 to OSHA within the twenty-four hours of the related incident in that the employer reported the amputation to OSHA on September 15, 2017.
