This was most recently observed at 3838 S 108th St, Omaha, Nebraska, on or about September 22, 2017 where the power cord for the AIDA Punch Press was attached to the outside of a section of conduit and held in place with zip ties.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: The employer is failing to protect employees from electrical shock hazards by not ensuring adequate strain relief is used on electrical components. This was most recently documented at 3838 S 108th St, Omaha, Nebraska on or about September 22, 2017 where the controller cord for the AIDA Feeder machine was observed with deteriorated insulation causing a lack of strain relief.
