29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On or about September 22, 2017, the employer did not ensure that a competent person was performing frequent and regular inspections that could have recognized the fall from elevation hazard associated with employees accessing an unprotected roof edge without utilizing fall protection equipment.

29 CFR 1926.502(f)(1)(iii): Points of access, materials handling areas, storage areas, and hoisting areas shall be connected to the work area by an access path formed by two warning lines. On or about September 22, 2017, employees who were accessing/egressing an unprotected roof edge 11 ft. above surface level were walking between the access point and work area warning line system without guidelines installed for a path.
