SPS Roofing Systems LLC., failed to provide documentation demonstrating the abatement method and completion of the cited items.

Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.    On October 16, 2018 at 825 W Main Street in Hortonville, employees were working on a roof surface twenty and one half feet above surface level without utilizing fall protection equipment.

29 CFR 1926.503(a)(2)(iii):  The employer shall assure that each employee has been trained, as necessary, by a competent person qualified in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used;    On October 16, 2018 at 825 W Main Street in Hortonville, the employer did not provide required fall protection safety training for an employee who did not associate a fall from elevation hazard when working on a roof surface twenty and one half feet above surface level without required fall protection equipment.

29 CFR 1926.20(b)(2): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part; such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    On October 16, 2018 at 825 W Main Street in Hortonville, the employer did not ensure that a competent person was performing frequent and regular inspections that could have recognized the fall from elevation hazard associated with employees working on a roof surface with unprotected edges without utilizing fall protection equipment.     The SPS Roofing Systems, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(2), which was contained in OSHA inspection number 1265561, citation number 01, item 01a, and was affirmed as a final order on January 10, 2018, with respect to a workplace located at 2150 Holly road; Neenah, WI  54956.

29 CFR 1926.503(b)(1): The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.    On or about October 16, 2018 at 825 W Main Street in Hortonville, the employer did not provide written employee fall protection safety training certification for employees working on roof surfaces at heights greater than six feet.
