29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) Salting Processing Exterior; On or about September 22, 2017, and at times prior, the employer exposed employees to crushed by hazards in that forklift operator evaluation had not been provided to employees operating the Toyota forklift (Model 7FBEU15, SN 18180) to manipulate loads of peanut waste.
