29 CFR 1926.404(b)(1)(i): General. The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors.    a) Exterior of Building - Scaffold Left Side :  Employees were exposed to electric shock while using electric tools including but not limited to grinders that were not protected by a GFCI or an assured equipment grounding conductor program; on or about 9/9/17.

29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.    a) Exterior of Building - Scaffold Left Side: Employees engaged in masonry activities from a welded frame scaffold platform with only 4 planks were exposed to fall hazards of approximately 6 feet, on or about 9/9/17.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:    a) Exterior of Building -  Scaffold Left Side: Employees were exposed to fall hazards of approximately  6 feet while engaged in masonry activities from a welded frame scaffold that was not resting on a metal base plate, on or about 9/9/17.
