On or about: March 24, 2022 Location: 131 Mamaroneck Ave., White Plains, NY 10601  a) Employee engaged in masonry activities was accessing and egressing the third level scaffold platform that was bridged by (2) planks that crossed over approximately 4ft section to the wall. The employees were exposed to fall hazards of approximately 30ft to the ground below as a means of fall protection had not been utilized. Condition observed on or about 03/24/2022.
