29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. On or about: March 24, 2022 Location: 131 Mamaroneck Ave., White Plains, NY 10601 a) Employee engaged in masonry activities was accessing and egressing the third level scaffold platform that was bridged by (2) planks that crossed over approximately 4ft section to the wall. The employees were exposed to fall hazards of approximately 30ft to the ground below as a means of fall protection had not been utilized. Condition observed on or about 03/24/2022.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about: March 24, 2022 Location: 131 Mamaroneck Ave., White Plains, NY 10601 a) (Unprotected planked access) employees engaged in masonry activities climbed onto leading edge wall to access (2) planked bridged that crossed over approximately 4ft section to third level scaffold platform. The employees were exposed to fall hazards of approximately 30ft to the ground below as a means of fall protection had not been utilized. Condition observed on or about 03/24/2022.
