On or about September 27, 2017, and at times thereto, employees performed service and maintenance activities, including inspections, lubrication, cleaning, die adjustments and die changes, on mechanical power presses. The employer's specific lockout/tagout procedures for the #029 and #007 presses did not include all of the hazardous energy sources, such as stored mechanical energy and gravity forces.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  a. On or about September 27, 2017, the employer had not performed a periodic inspection of the lockout/tagout procedures for the #029 and #007 mechanical power presses in order to ensure authorized employees can perform the required steps and to correct any deviations or inadequances.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  a. On or about September 27, 2017, and at times prior thereto, employees were exposed to crushed by hazards while performing die changes and adjustments on mechanical power presses. The employer did not ensure that the exposed employee implemented energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect the employee from machine servicing hazards.

29 CFR 1910.217(c)(1)(i): It shall be the responsibility of the employer to provide and insure the usage of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on a mechanical power press. See Table O-10.   a. On or about September 20, 2017, the employer did not ensure that a damaged plexiglass barrier guard enclosing the point of operation was repaired prior to allowing an employee operate the Johnson #55 Press, Machine #29. The unprotected portion of the press exposed the employee to an amputation hazard.

You must enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred.  Located at the Jones Metal Products Company facility at 200 North Center Street in West Lafayette, Ohio:    a. As of September 27, 2017, the employer did not ensure recordable workplace injuries that occurred in 2017 were recorded on an OSHA 300 Log within seven days of notification.     1.     On September 13, 2017, an employee received a laceration to the thumb and received medical treatment.
