29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: a. On or about February 1, 2019, located in the Stamping Department, employees performed service and maintenance activities, including inspections, lubrication, cleaning, die adjustments and die changes, on mechanical power press #29. The employer's specific lockout/tagout procedures did not include a specific statement of the intended use of the procedure; the specific rules and techniques for shutting down, isolating and blocking/securing the press; specific procedural steps for placement and removal of lockout/tagout devices and the responsibility of each. The procedure did not include specific requirements to test the press to determine and verify the effectiveness of lockout devices.

29 CFR 1910.147(c)(6)(i)(C): Where lockout is used for energy control, the periodic inspection shall include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected. a. On or about February 8, 2019, located in the Stamping Department, the employer did not ensure there was a review of the energy control procedures between the inspector and each employee for Press #29 and Press number #007.
