29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, were not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures. Water heater Enclosure Next to Trailer: Electrical equipment and parts, cardboard boxes, insulation materials, construction materials were scattered throughout working and walking areas.

29 CFR 1926.404(b)(1)(i): General. The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors. Outside the Laboratory: Ground fault circuit interrupters that were not provided to employees who used a portable generator and portable electrical tools outdoor exposed to rain. The tools were powered through an extension cord missing a ground prong. Employees were exposed to electrical shock hazards.

29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. Outside the Laboratory Building: There was no ground prong on the extension cord that powered electrical tools inside the building. The cord was plugged into portable generator outlet. Employees were exposed to electrical shock hazards.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables were not protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. Outside the Office Trainer: One yellow flexible extension cord was not protected from damage as it lay on the metal landing on steps and a wooden door frame. The cord was exposed to being pinched between the door and the metal frame. Employees were exposed to electrical shock and burns.

Flexible cord was not connected to devices and fittings so that strain relief was provided which will prevent pull from being directly transmitted to joints or terminal screws. Outside the Lab: One orange flexible extension cord was not connected to the plug that was used to power portable electrical tools inside the laboratory building. The cord was exposed to rain and exposing one employee to electrical shock and burns.

29 CFR 1910.1200(h)(1): Employers did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets. Outside the Laboratory: An electrician who handled and used hazardous chemicals, such as gasoline, oatey dark thread cutting oil, and rectorseal dark thread cutting oil at this job site. Effective information and training on the hazards related to the hazardous materials employee was working with had not been conducted at the site. The California Constructors was previously cited for a violation of this occupational safety and health standard or its equivalent standard 19101200(H)(1), which was contained in OSHA inspection number 1129994, citation number 1, item number 1 and was affirmed as a final order on July 24, 2016, with respect to a workplace located at Us Navy Lodge, Building 3526Naval Base San Diego.
