29 CFR 1910.333(b)(2)(ii)(B): The circuits and equipment to be worked on were not disconnected from all electric energy sources: a) Building 47, 30 Curry Avenue, Canonsburg, PA 15317: On or about September 28, 2017, a potential transformer that was contained within a substation style transformer (serial number R-08718-1) was not disconnected from all electric energy sources. A work bench used to provide power a load tap changer (LTC) motor also provided power to the potential transformer. The potential transformer converted the power to approximately 2000 volts on the high voltage side. Employees performing ratio and resistance testing were exposed to the hazards of electric shock and electrocution while working in close proximity to the potential transformer bushings.

29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag were not placed on each disconnecting means used to deenergize circuits and equipment on which work was to be performed: a) Building 47, 30 Curry Avenue, Canonsburg, PA 15317: On or about September 28, 2017, the employer did not ensure that locks and tags were used on each disconnecting means used to denergize circuits and equipment. Locks and tags were not used when employees installed and removed test leads during ratio and resistance testing. Employees were exposed to the hazards of electric shock each time they installed or removed the test leads.

29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments: a) Building 47, 30 Curry Avenue, Canonsburg, PA 15317: On or about September 28, 2017, the employer did not ensure that each employee assigned to perform ratio and resistance testing on a substation style transformer (serial number R-08718-1) were trained in and familiar with safety related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments. The employees were exposed to the hazards of electric shock and electrocution while working in close proximity to energized components housed within the equipment. Pennsylvania Transformer Technology, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.332(b)(1), which was contained in OSHA inspection number 1156986, citation number 1, item number 2 and was affirmed as a final order on August 18, 2016, with respect to a workplace located at 30 Curry Avenue, Canonsburg, PA 15317.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: a) Front of Assembly Finish Office, Building 47, 30 Curry Avenue, Canonsburg, PA 15317: Observed on or about October 5, 2017, a flexible cable being used to supply power to the hot oil machine was not connected to an outlet such that tension would not be transmitted to the joints or terminal screws. Employees using the flexible cable and plug were exposed to the hazard of electrical shock.
