29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Oven Feeder, Table Talk Pies, 120 Washington St., Worcester, Massachusetts; On and before October 11, 2017, employees were standing at the edge of the feeder to monitor the operation and reaching into the machine to adjust or pick up pies that had turned over or needed to be moved. The feeder bar that rotated to the edge of the oven loader was not guarded and employees were not protected from struck by and puncture hazards.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Table Talk Pies, 120 Washington St., Worcester, Massachusetts; Authorized personnel were not utilizing documented energy control procedures to ensure potentially hazardous energy sources were put into a safe condition prior to performing work on machinery in a commercial bakery. a. On September 25, 2017 a maintenance employee was troubleshooting a problem with the chocolate pump. He attempted to adjust the pneumatic valve without locking the machine out. b. On October 11, 2017 an employee was observed cleaning the feeder to the line one oven while the feeder arms were operating. The lock out procedure was not utilized for this equipment while performing the cleaning operation. Table Talk Pies was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(I), which was contained in OSHA inspection number 1184995, citation number 2, item number 1 and was affirmed as a final order on April 14, 2017 , with respect to a workplace located at 28 Bowditch Dr., Shrewsbury, Massachusetts. Table Talk Pies was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(I), which was contained in OSHA inspection number 1114302, citation number 1, item number 1 and was affirmed as a final order on April 4, 2016 , with respect to a workplace located at 28 Bowditch Dr., Shrewsbury, Massachusetts. Table Talk Pies was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(I), which was contained in OSHA inspection number 1044421, citation number 1, item number 1 and was affirmed as a final order on April 22, 2015 , with respect to a workplace located at 120 Washington St., Worcester, Massachusetts.
