29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  Masterack, LLC -1 Waters Drive, Social Circle, GA - (Throughout Plant) - On or about 10/04/17, employees were exposed to sudden energization or start-up of multi-energy source machines.   The employer did not have machine specific written procedures for locking out/tagging out the multi-energy machines to safe guard employees on machinery or equipment. Equipment includes, but is not limited to, Amada Laser & Automation, Verson Coil Press 17, and Amada Turret 66.  The company's written lockout/tagout specific procedures were found with the following deficiencies:  a)A specific statement of the intended use of the procedure;  b)Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;  b)Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them;  c)Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Masterack, LLC -1 Waters Drive, Social Circle, GA -  Employees were exposed to a point of operation hazard.  An employee operating the lathe machine and his shirt and arm was caught into the unguarded rotating part (shaft), on or about 9/21/17.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours  Masterack, LLC-1 Waters Drive, Social Circle, GA - On September 21, 2017, an employee was hospitalized after his shirt and arm was caught in a lathe machine. The employer notified OSHA on September 26, 2017. The employer did not notify OSHA of the hospitalization within 24 hours.    No abatement certification or documentation is required for this item.
