Abatement certification was not received for the following violations:                a.  Citation 1 Item 1, and              b.  Citation 1 Item 3, and             c.  Citation 2 Item 1.

29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork:  On or about October 2, 2017, Danny Miller dba Miller Carpentry had employees exposed to 11.5 foot fall hazards, while working from a supported scaffold lifted by a rough terrain forklift, on a residential construction site in Champaign, Illinois.   The employer did not secure the supported scaffold to the rough terrain forklift by a chain or some other secure method.

DANNY MILLER DBA MILLER CARPENTRY WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 1926.501(B)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER: 1086438, CITATION NUMBER 1, ITEM NUMBER 1A AND WAS AFFIRMED AS A FINAL ORDER ON JANUARY 12, 2016, WITH RESPECT TO A WORKPLACE LOCATED AT 309 CAPITOL DRIVE, SAVOY, IL 61874.
