29 CFR 1910.157(b)(1): A written emergency evacuation plan, meeting the requirements of 29 CFR 1910.38, was not developed and implemented  where the employer elected to evacuate employees upon sounding of the a fire alarm:    a) Facility-Wide - The employer elected to evacuate employees upon sounding of a fire alarm. The employer did not develop and implement an emergency action plan which complied with 29 CFR 1910.38. On or about October 4, 2017, employees were not aware of procedures for reporting a fire or other emergency; procedures for emergency evacuation, including type of evacuation and exit route assignments; procedures to be followed by employees who remain to operate critical plant operations before they evacuate; procedures to account for all employees after evacuation; procedures to be followed by employees performing rescue or medical duties; the name or job title of every employee who may be contacted by employees who need more information about the plan or an explanation of their duties under the plan.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:      a)  Packing Area - On or about October 4, 2017 two (2) 480v disconnect switches were blocked by multiple boxes filled with tee-shirts, a parked forklift with a pallet of cardboard, and an empty skid.    b) Mug Area (Dance Studio) - On or about October 4, 2017 a 60 amp/500 volt disconnect switch was blocked by a UV curing machine.
