29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  (a)  PACKING AREA - On or about February 13, 2019, three (3) 480v disconnect switches and one (1) 480v breaker panel were blocked by multiple shelving units, plastic storage totes, and a step stool.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.  Effective Plan Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.303(g)(1), which was contained in OSHA inspection number 1268536, citation number 1, item number 2, and was affirmed as a final order on December 13, 2017, with respect to a workplace located at 47 Cherry Street, Seven Valleys, PA 17360.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    (a)  PACKING/SILK AREA - On or about February 13, employee(s) were not trained on the hazards of the chemicals that they work with, including but not limited to Xenon Products Spot Buster.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records. 29 CFR 1910.1200(f)(6): Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with either the information specified under paragraphs (f)(1)(i) through (v) of this section for labels on shipped containers; or, product identifier and words, pictures, symbols, or combination thereof, which would have provided at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, would have provided employees with the specific information regarding the physical and health hazards of the hazardous chemical:    (a)  DIRECT TO GARMENT (DTG) AREA - On or about February 13, 2019, the employer did not ensure that all containers of hazardous chemicals were labeled with the product identifier and the hazard warnings.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
