29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a)Gil Haugan Construction, Inc. at 5217 South Louise Avenue, Sioux Falls, SD 57106: On and prior to October 6, 2017, the exposing employer did not ensure employees were protected from injury, in that two employees installing sheathing on the roof of a commercial property and were not utilizing a fall protection system. This condition exposed employees to a fall hazard of approximately 12 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a)Gil Haugan Construction, Inc. at 5217 South Louise Avenue, Sioux Falls, SD 57106: On and prior to October 6, 2017, the exposing employer did not ensure employees were trained to recognize fall hazards associated with the roofing sheathing activities. Employees were engaged in sheathing the roof of a commercial building and had not received adequate fall protection training prior to beginning the work. This condition exposed employees to a fall hazard of approximately 12 feet to the concrete below.
