29 CFR 1926.1203(e)(2)(ix): The employer did not verify that the confined space was safe for entry and that the pre-entry measures required by paragraph (e)(2) of this section were taken, through a written certification that contains the date, the location of the space, and the signature of the person providing the certification. The certification was not made before entry and was not made available to each employee entering the space or to that employee's authorized representative. a) Lincoln Avenue, Whitehall - On or about October 6, 2017, employees entered a sanitary sewer, a permit required confined space, without certifying that the space was safe for entry and that the pre-entry measures required by paragraph (e)(2) of this section had been taken.
