29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) 1800 Arch Street, Philadelphia, PA 19103, Level B-1 - The employer did not ensure that the path to ground was permanent and continuous from equipment, in that the ground prong was missing from the power plug on a Dewalt model DWM120 5" Deep Cut Variable Speed Band Saw, serial number 170244, while an employee used it, thus exposing the employee to an electrical shock hazard. This condition was observed on or about September 15, 2017.
