29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed: a) Viking Tool & Gage, Inc. in Conneaut Lake, PA. On or about October 11, 2017, employees conducted servicing and maintenance work on machines and equipment that included but was not limited to CNC machines. Annual periodic inspections of the lockout/ tagout procedures (energy control procedures) had not been conducted.

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by rotating parts: a) Viking Tool & Gage, Inc. in Conneaut Lake, PA. On or about October 11, 2017, the rotating tool changer of the Moriseiki Sh-403 CNC machine was not guarded to protect the employees from having fingers and/ or hands in the machine during operation.
