Viking Tool & Gage, Inc., Press area of the facility(Cooper assembly machine#52-37), on or about December 1st, 2017 and times prior thereto: An employee performing work operations at the Cooper assembly machine in the press area of the facility was exposed to carbon monoxide at a time weighted average (TWA) of 58 parts per million (ppm), which is approximately 1.2 times the Permissible Exposure Limit (PEL) of 50 ppm. The exposure was derived from one sample collected over 258 minutes and assumed zero exposure for the remaining work shift. b.) Viking Tool & Gage, Inc., Press area of the facility, on or about December 1st, 2017 and times prior thereto: An employee operating a Komatsu brand powered industrial truck (No identifiable serial/model #) was exposed to carbon monoxide at a time weighted average (TWA) of 56 parts per million (ppm), which is approximately 1.1 times the Permissible Exposure Limit (PEL) of 50 ppm. The exposure was derived from one sample collected over 254 minutes and assumed zero exposure for the remaining work shift. c.)
