Location: 99 Hudson St., Jersey City, NJ.    a) Employees used a SkilSaw circular saw for wood cutting that was powered by an electrical cord that was missing the ground pin.  The path to ground was not permanent and continuous; on or about 10/3/16.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around the holes;    Location: 99 Hudson St. Jersey City, NJ    a) The DOKA cacoon system's access stair had a hole/gap that was not protected (from access stair to floor slab) thereby exposing employees to a potential fall hazard to the floor below; on or about 10/13/17.    b) The floor penetration/hole used for the concrete pump's hose on the 10th and 11th floor was not protected thereby exposing employees to a potential fall hazard to the floor below; on or about 10/3/17.    c) The DOKA cacoon system's flaps used by employer did not match up with the edge of the floor slab creating gaps/holes thereby exposing employees to a potential fall hazard to the floor below; on or about 10/3/17.

Location: 99 Hudson St., Jersey City, NJ.    a)  Employer did not provide a stairrail system for stairways used by employees to access their work zones thereby exposing employees to a potential fall hazard of greater than 10ft; on or about 10/3/17.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    Location: 99 Hudson St., Jersey City, NJ.    a) Employees working on concrete form stripping operations were not protected while walking/working near unprotected edges when their retractable lanyards were detached/disengaged from the anchorage point (i.e., shoring posts) thereby exposing employees to a potential fall hazard of approximately 80 ft. to the ground below; on or about 10/3/17.    b) Employees were observed working/walking near unprotected edges that were missing guardrail sections thereby exposing employees to a potential fall hazard ranging from approximately 60ft to 130ft to the ground below; on or about 10/13/17.    The Forsa Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1) which was contained in OSHA inspection number 1155827 citation number 2, item number 1 and was affirmed as a final order on 10/17/2016, with respect to a workplace located at 430 Marin Blvd., Jersey City, NJ.
