29 CFR 1910.333(b)(2)(ii)(A): Safe procedures for deenergizing circuits and equipment were not determined before circuits and equipment were denergized: a) On or about 10/05/2017 - Cast house basement, unit #4 converter, employees conducting electrical maintenance on a converter did not have procedures in place to ensure that all components in the converter cabinet were deenegerized prior to performing maintenance.
