(a) South side of house on roof - On or about October 12, 2017 an employee was carrying bundles of roofing shingles up house roof using a extension ladder as a crawling board that was not secured to the roof to keep it from dislodging.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  (a) Around job site - On or about October 12, 2017 the employer failed to provide ladder training for his employees that were using the ladders to gain access to the roof and carry material to the top of the roof. All 4 ladders used by employees and employer were not placed in accordance to OSHA regulations.
