29 CFR 1926.501(b)(4)(i):  Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:      a)  On October 13, 2017, employees were exposed to falls of approximately 11 feet while working around unprotected floor openings.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:      a)  On October 13, 2017, employees performing masonry work were exposed to fall hazards from the 4th story deck of the building.      S.M. Masonry Walls Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1) which was contained in OSHA inspection number 1067242, citation 2, item 1, and was affirmed final order on October 5, 2015, with respect to a workplace located at 3701 N. Ashland Ave., Chicago, IL 60613.      S.M. Masonry Walls Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1) which was contained in OSHA inspection number 110926, citation 1, item 6, and was affirmed final order on February 29, 2012, with respect to a workplace located at 3656 N. Ashland Ave., Chicago, IL 60613.      No abatement certification or documentation is required for this item.
