29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Rewind slitter, SN#115470, 530 Main Street Clinton, MA: On or about October 12, 2017 one or more methods of guarding was not provided to protect the operator or other employees manually tensioning products during rewinding/slitting operations from the following hazards: (a) Point of operation hazard, located on second frame between tension roller and unprotected edges of slitter knives. (b) Pinch point hazards, located on second and third frame, tension rollers where fabric runs under and behind rollers creating an in running nip point. (c) Rotating part hazards, located on top of first and third frame, wind and rewind rollers not guarded exposing employees to rotating shafts.
