This was most recently documented at 4717 Chennault St, Bellevue, Nebraska on or about October 13, 2017 when employees were observed installing shingles to a residential structure while working greater than 6 feet above the ground without fall protection. Lisandro Lucas was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 952080, citation number 1, item number 2, and was affirmed as a final order on January 31, 2014, with respect to a workplace located at 1021S.W.
