(a) L&R 25 Ton Over Head Crane (Serial No. 12745); West Bay South Corner: On or about October 16, 2017, employees would disengage from remote controlled crane controls to scrape the slag off the edge of a suspended steel plate load, exposing them to a crushing and struck-by hazard.
