29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent: 862 West Ridge Road, Gainesville, GA - Refinery Area - An employee was injured while attempting to evacuate the Deodorizer Tower Area while working on a heat exchanger. The nearby door was blocked with scaffolding, a ladder, etc. which contributed to the injury. The employer did not have signs or other marking that provided direction to a safe evacuation route for exiting the refinery's Deodorizer Tower Area, on or about October 11, 2017. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(c)(4)(ii)(B) The energy control procedures did not clearly and specifically outline the techniques to be utilized for the control of hazardous energy, in that specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy were not provided: Cargill Incorporated, 862 West Ridge Road, Gainesville, GA- Refinery Area- Lockout / tag out (LOTO) procedures used for flushing/draining and locking out the Heat Exchange (Deodorizer) Final Cooler did not provide clear and specific steps for shutting down and isolating the heat exchanger to control hazardous thermal energy and hot water pressure, on or about October 11, 2017. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
