29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):      a) On or about August 25, 2017, the employer did not assure the point of operation on the crimping machine at the table top in the manufacturing area was guarded.  An employee was injured when they reached into the machine and activated it resulting in an amputation of their left index finger tip.    b) On or about October 18, 2017, the employer did not assure the point of operation on the copper tube rolling machine in the manufacturing area was guarded.  An employee was injured when they were feeding a tube into the machine and activated it with a foot pedal pinching their right index finger resulting in an amputation.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.         1.) On or about October 18, 2017, the employer failed to report an employee amputation injury.  On August 25, 2017, an employee operating a crimping machine at the table top in the manufacturing area sustained an amputation injury; the employer did not report the amputation within 24 hours.
