29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Raytheon Meat Grinder - On or about 10/14/2017, an employee reached under the hood at the product discharge and came in contact with the moving parts of the grinder, resulting in a finger amputation. b) Tomahawk Line 5 - On or about 1/20/2018, the conveyor belt coming out of the tunnel between the metal conveyor and the plastic conveyor had a pinch point, which resulted in a finger amputation.
