On or about January 6, 2014, and times prior thereto, in the engine room, the path to two exit doors were blocked exposing employees to the hazards of entrapment and fire hazards. a. 29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces. On or about January 6, 2014, and times prior thereto, on the south west of the facility, the employer did not place a permit required confined space signs on the waste water areas exposing employees to the hazards of entrapment and asphyxiation hazards.
