29 CFR 1910.219(d)(1): Pulleys with parts seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): (a) General Shale Brick, Inc. at 1845 West Dartmouth Ave., Denver, CO 80110: On and before October 19, 2017, the employer did not ensure that the tail pulley used in the thin brick feed line was adequately guarded against accidental contact. This conveyor was used to haul debris from two saws that cut bricks into smaller pieces. An employee was injured when his arm was caught in the winged pulley, resulting in fractures and lacerations. This condition resulted in exposure to an amputation hazard.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) General Shale Brick, Inc. at 1845 West Dartmouth Ave., Denver, CO 80110: On and before October 19, 2017, the employer did not ensure that the thin brick feed line conveyor belt was adequately guarded. The lower return section of the belt was not guarded at the shear points where the belt passes through fixed vertical support structures. This condition resulted in exposure to an amputation hazard.
