29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  At the residential construction site located at 7935 Holmes Road in Kansas City, Missouri:  Employees were exposed to struck-by hazards. Employees were engaged in roofing activities with the use of pneumatic nail guns and were not provided eye protection while installing roofing materials.

Employees were operating a ROLAIR compressor, serial # 11061782 in which the guard for the belt and pulley was removed.

Employee were performing work on a steep roof and were not protected from falling by the use of guardrails, safety nets, or personal fall arrest systems. Fall distance measured from the eve of the roof to the ground below was 18.0 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a residential worksite located at 7935 Holmes Rd, Kansas City, Missouri: Employees were exposed to fall hazards. The employer failed to provided safe access to an upper landing for employee. Employees were observed using a portable ladders to gain access to a roof, and that ladder was only placed 2 feet above the upper landing that was being accessed.
