29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:     Maintenance/Housekeeping:  Employees were not trained on the new label elements (e.g., pictograms and signal words) and the new SDS formation by December 2, 2013, as required by the revised Hazard Communication Standard.  Employees use hazardous chemicals such as bleach, Ecolab cleaning products, gasoline, and pool maintenance chemicals.

29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors:   Corridor, Chlorine Storage Area:  Three gasoline storage containers  (1-5 gallon volume) were either uncapped or had cracks at the base of the screw tops when not in use.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):   Maintenance/Housekeeping: The employer has provided employees with personal protective equipment (PPE) such as gloves and eye protection to be worn when performing maintenance and housekeeping duties such as mixing and using cleaning products or adding pool chemicals.  The use of PPE is a job requirement.  A hazard assessment has not been performed to include the following elements:                    1.  Select and have each affected employee use the types of PPE that will protect them from the hazards identified in the hazard assessment;  2.  Communicate the selection decisions to each affected employee and;  3.  Select PPE that properly fits each affected employee.
