Shop Area & Outdoors - On or about and times prior to October 19, 2017, the employer allowed employees to operate a Toyota forklift (model: 7FGU25), a Caterpillar forklift (model: GP30K), a JCB forklift (Type: 550-140), and a Toyota forklift (model: 7FBCU25) without the use of the seatbelt, thus exposing employees to an ejection hazard.

Shop Area - On or about and times prior to October 19, 2017, a Toyota forklift (model: 7FGU25) was operated with a damaged fork and an inoperable seatbelt, thus exposing employees to a struck-by and ejection hazard.    b). Shop Area - On or about and times prior to October 19, 2017, a Caterpillar forklift (model: GP30K) was operated with a damaged fork and inoperable horn, thus exposing employees to a struck-by hazard.    c). Outdoors - On or about and times prior to October 19, 2017, a JCB forklift (Type: 550-140) was operated with an inoperable seatbelt, thus exposing employees to an ejection hazard.
