29 CFR 1926.501(b)(12):  Each employee engaged in the erection of precast concrete members (including, but not limited to the erection of wall panels, columns, beams, and floor and roof "tees") and related operations such as grouting of precast concrete members, who is 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:    location: 9th floor  on or about: 10/18/17    a) Employees engaged in precast construction activities, were not protected from falling. The employees were exposed to a fall of up to 9 feet 4 inches.     NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.

All protruding reinforcing steel, onto and into which employees could fall, was not guarded to eliminate the hazard of impalement:    Location: 9th floor  on or about: 10/18/17    a) Employees were working beside approximately 20 unguarded rebar. 29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with one stairrail system along each unprotected side or edge:    location: stairs between 8th and 9th floors  on or about: 10/18/17      a) Employees were using the stairs to access the 9th floor while installing precast planks. The open side of the stairs were not provided with stairrails.        NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.
